It is becoming increasingly important that we follow best practices to maintain appropriate reimbursement. Below are some of the best practices for reimbursement:\u00a0<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

  • Chief Complaint \u2013 a concise statement that describes the symptom, problem, condition\u00a0or reason for the encounter. It is typically stated in the patient\u2019s own words. A word of\u00a0<\/span>caution: \u201cfollow up\u201d is not a reimbursable diagnosis.<\/span><\/li>
  • Old records reviewed must have an independent note of your review and findings or a\u00a0comparison noted. Date of prior chart reviewed and your signature is a must.<\/span><\/li>
  • Diagnosis must have specificity and detail to support both the encounter and all\u00a0procedures: i.e., location\/laterality, acute vs. chronic.<\/span><\/li>
  • Smoking cessation is a three-minute reimbursement opportunity. Documented\u00a0diagnosis should support medical necessity for counseling provided.<\/span><\/li>
  • EKGs must have three of the following elements to code and bill: rhythm, rate, axis,intervals, ST wave changes, comparison to previous EKGs, or presence or absence of\u00a0<\/span>ectopy.<\/span><\/li>
  • Lastly, a thorough and well-documented medical record must include all required\u00a0elements (chief complaint, ROS, HPI, PFSH, PE, MDM) as well as thorough procedure\u00a0<\/span>notes with a diagnosis for it to be reimbursement friendly (emergent vs. non-emergent). <\/span>
    • For example, acute abdominal pain or acute chest pain. Acute is the important\u00a0<\/em>word here.<\/em><\/li><\/ul><\/li><\/ul>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<\/div>\n\t\t","protected":false},"excerpt":{"rendered":"
